School Wellness Program
The School Wellness Program (SWP) is a community-based program with the aim of increasing access to basic health care for Michigan children, teens and young adults. The SWP sites are located in schools and offer clinical services, referrals and health education provided by a Registered Nurse (RN) and a licensed mental health provider.
Jackson County Health Department has collaborated with Concord Community School District to offer on-site services to all PK-12th grade Concord students. Parents/Guardians must provide consent for their minor children for SWP services, with certain exceptions required by federal and Michigan laws*.
The School Wellness Program operates through the Jackson County Health Department. Your student’s health information is protected by HIPAA, and is not shared outside of SWP staff without signed permission. The only exceptions where health information is shared with school staff without signed permission include service coordination, care management on a need to know basis, medical or mental health emergencies, safety concerns, and bullying.

General Services Provided:
- Medicaid/MIChild Enrollment Assistance
- Community Resources and Referrals
- Risk Assessment (for more information or additional details, please contact a staff member)
- School Staff and Community Education/Training about Health Issues
Nursing Services Provided:
- Screening/nursing assessment of general health
- Assessment of minor injury and illness, administration of over-the-counter medication
- Coordination of care and chronic disease management with the school, primary care and/or specialty provider
- Immunization assessment and referral
- Nutrition and health promotion/education
- School staff education/training about health issues
- Communicable disease management
Mental Health Services Provided:
- Counseling sessions
- Group Counseling
- Crisis intervention
- Diagnosis of mental health conditions
- Assessment of risk behaviors
- Risk reduction counseling
What Services are NOT Provided
- Diagnosis of medical conditions
- Administration of daily prescribed medications
- Abortion advice, referrals or services
- Family planning medications or devices
- Immunizations
- Prescription of medications
“Health Hive”/Youth Advisory Committee
The Health Hive is a group of students committed to improving wellness education and awareness among their peers. Students that are a part of the Health Hive have the opportunity to:
- Work on wellness education projects/events
- Build leadership skills
- Make a difference

*Current Michigan Law allows for confidential services to minor students in these areas:
Referral for suspicion of child abuse and/or neglect
Emergency care/first aid
12 years or older: Referrals and education regarding sexually transmitted diseases, including HIV. Referrals and education regarding family planning. Referrals and education regarding pregnancy care. Substance abuse counseling and referral.
14 years or older: Limited outpatient mental health services not to exceed 12 visits or four months and not to include any medications. Crisis interventions including student: threatening suicide, life is threatened, threatening harm to self or someone else, self-harm or thoughts to harm self.
